EVERGREEN FAMILY CLINIC – MEDICAL REPRESENTATIVE POLICY
• Appointment allotment is subject to clinic approval and availability.
• Only one official meeting per calender month is permitted for the same SKU / brand group, irrespective of whether the visitor is a Medical Representative, Area Manager, Regional Sales Manager, Zonal Sales Manager, Product Manager, or any other company representative.
• Medical Representative meetings are conducted only during the designated clinic MR interaction window of 9:15 AM to 9:30 AM.
• A maximum of only 5 company/division representatives may be allotted appointments on a single working day.​
• Submission of this form does not guarantee an appointment.
• Unscheduled promotional visits are discouraged and may not be entertained.
• Representatives are responsible for keeping their SKU list updated and as accurate as exact brand names. Brands transferred to another representative, division, or company should be removed from the registered SKU list.
• Any patient-support programs, discounts, camps, or promotional activities offered must comply with applicable laws, regulations, and company policies.
• Evergreen Family Clinic reserves the right to approve, defer, reschedule, or decline meeting requests without assigning a reason.
• Any prescription copies or clinical information shared by Evergreen Family Clinic will be anonymized/redacted and must be used strictly for approved professional purposes.
